The following forms are an important part of this ministry.  We are concerned for your student’s safety just as you are.  Franklin Christian Church keeps some forms on file.  For the Medical form, we do like to update it every year.  If you have any questions about these forms, please see Jeremy Stevenson or Christy Anderson.

FRANKLIN CHRISTIAN CHURCH

1.  Medical Form We must have this form on file to attend any FCC event.  This is the form that needs to be updated every year.

2. Youth Group Rules Read, understand, sign, & return. This is an overview of the policies for FCC Youth Group.

3. Media-Release-Form This form gives permission for your photo/video to be used in a production of Franklin Christian Church.

4. BELIEVE  2012 Sign up sheet. Print this page.   Sign it and return it to Jeremy in order to register for the trip.  In order for your Son/Daughter to go to Believe, they will also have to fill out the medical form below.
